To start our exercise physiology program with Medicare rebates, your local GP will need to complete a Chronic Disease Management Plan.
To qualify for this plan, your doctor must deem you as a complex case (this means that you are presenting with a number of conditions in which a higher level of care is needed). This plan is designed to maximise your treatment for your condition(s) by collaborating with your Doctor and/or Specialist and an Allied Health Professional.
